Dear Kaiser permanente,

I am sending you this written notice to request cancellation of my insurance policy effective 10/01/2010. I would appreciate you sending me written confirmation within 30 days that the cancellation has been put into effect. Please refund the unused portion of my policy premium, and cease charging my bank account for payment of monthly premiums.

Note: Do I need include the reason, cause I don't have money to pay for it. I just got excepted and is been charged and the effective date is oct 1, 2010.
